“Tea and a Talk” Dialogue Amongst Teams Working on Medication Reconciliation: A Safer Healthcare Now! Virtual Interactive Series

We have heard from many of you that there is a strong desire to hear what others are doing for medication reconciliation. Following a survey that was conducted in the fall of 2009, we heard an interest about:

  • How people are successfully implementing medication reconciliation in acute and ambulatory care settings; and
  • Measurement strategies.

These sessions above will be a facilitated conversation amongst teams to share what they are doing at their site, in their area, successes, challenges, ideas, and hopefully some butterflies that you can take home after the session!
