Patient Safety & Learning System

The BC Patient Safety & Learning System (BC PSLS) is a web-based safety event reporting tool that is currently being implemented across BC.  The goal of the BC PSLS is to establish a system to help healthcare providers make healthcare safer for patients in BC.  The BC PSLS provides effective tools to facilitate reporting and follow-up, and automated processes that save time and increase efficiency.

The BC PSLS fosters and supports:

  • Common processes, language, definitions and reporting standards;
  • Prompt identification and follow-up of safety events and hazards;
  • Improved communication about adverse events;
  • Event analysis to determine root causes;
  • Timely feedback to “close the loop”;
  • Identification of preventative measures and sharing of information to reduce risks;
  • Better information about the state of care within facilities;
  • A just, non-punitive, reporting and learning environment; and
  • A culture of safety

The BC PSLS will benefit patients by providing healthcare staff with tools to analyze trends and current practices toward developing strategies to improve patient safety and quality of care at BC healthcare facilities. The project is the first of its kind in Canada.
