Jill Veenendaal, Quality Leader for Measurement, BC Patient Safety and Quality Council started off with a general review of the Clinical Care Management measurement requirement for med rec in long-term care. 

Fruzsina Pataky (Medication Safety Coordinator) & Karin Trapnell (Regional Project Manager – Med Rec) discussed the Vancouver Coastal Health/Providence Health med rec team's struggles and successes in creating a data collection plan for long-term care.

Also featured: Dr. Trevor Janz shared his experience admitting patients to long-term care.

A national update on medication reconciliation, with a focus on long-term care, was provided by Jennifer Turple, Medication Safety Specialist -- Consultant with ISMP Canada.

Anita Lo, Med Rec Facilitator for Fraser Health, shared her team's approach to medication reconciliation at admission to long-term care as they began their journey earlier this year.
