Disclosure of Adverse Events

The Disclosing Unanticipated Medical Outcomes (DUMO) approach is the education strategy for disclosure adopted by health authorities in BC.  The BC Patient Safety & Quality Council collaborates with the Healthcare Protection Program to support DUMO training in the province.

This Forum provided participants with an opportunity to exchange knowledge and share their perspectives on the topic of disclosure. Invitations were extended to key leaders for quality, safety and medicine in the province, including health authorities, the CMPA, HCPP, College’s and others. Click here to download the invitation and agenda (pdf-780Kb).

Disclosure Policy, Tools and Communication

In July 2007, the BC Patient Safety Task Force and the Healthcare Protection Program released the Provincial Guidelines for Policy Related to the Disclosure of Adverse Events (pdf-65kb). To support the work of health authorities around disclosure, the BC Patient Safety Task Force sponsored situation manager and workshop leadership training in the Institute for Healthcare Communication’s Disclosing Unanticipated Medical Outcomes (DUMO) program.

A Checklist for Disclosure Team Discussion (pdf-379kb) was also developed to identify tasks to be completed or delegated during a meeting of the disclosure team prior to speaking with the patient and his or her support person(s).

The Task Force also created a number of communication materials and tools for health care providers, as well as an information pamphlet for patients and their families, Conversations When Things Go Wrong:  Information for Patients and Families (pdf-379kb).

The BC Patient Safety & Quality Council will continue the work of the Task Force by supporting DUMO training through the provision of the required DUMO materials and communication materials to health authorities.
