Clinical Care Management Data Guide
The Data Guide contains the definitions and scope for the measures associated with each Clinical Topic Area. The foundation of this guide is the acknowledgement that improvement happens over time, and that data should be presented and analyzed using time series in order to best identify these changes.
The Guide was approved by the CCM Steering Committee on October 28, 2011. Additional content was approved on April 27, 2012. As updates are made to the Guide, they will numbered and made available here. [Please note that the most recent version (2012v.1.0.4) is available for download via the link below.]

Setting Targets
In order to ensure that your site continues to aim to improve quality, it is important to set targets for improvement. Each Clinicial Expert Group associated with a clinical topic area will be asked to set provincial targets for process measures. Targets set for outcome measures must be set at a local level in order to account for local context.
The CCM Steering Committee has approved a document written by the Measurement Working Group entitled "Principles for Setting Targets" that will assist local sites and Clinical Expert Groups to develop their targets. Click HERE to download the document.
