Medication Card
Your health care providers need to know all the medications you are taking to provide you with safe health care.
When you see a doctor, nurse and pharmacist, it is important for you to take with you a list of prescription medications and non-prescription medications you are taking. This also includes vitamins or herbal remedies. You can use the It's Good to Ask Medication Card (pdf-511Kb) to make this list.
