Faculty
We have an excellent group of dedicated experts who are both passionate and knowledgeable about their areas of expertise related to improving the quality of care. These faculty members will provide direct teaching and be available for advice to program participants. As well, several faculty will also serve as mentors who will be paired with a participant at the first residency program, to provide ongoing one-to-one.

Lloyd Provost, MSc, Statistics Associate, Associates for Process Improvement With an education in statistics, Lloyd supports organizations to increase their capacity to continually learn and improve. His experience includes consulting in planning, management systems, measurement, planned experimentation, and other methods for improvement of quality and productivity. In his work with leaders of organizations, Lloyd uses the Associates in Process Improvement (API) Quality as a Business Strategy template for alignment and focusing of improvement. Lloyd is a senior fellow of the Institute for Healthcare Improvement (IHI) where he serves as an improvement advisor supporting collaborative and other improvement programs. Lloyd also supports health care improvement in developing countries, the National Initiative for Children’s Healthcare Quality (NICHQ ) and The Center for Children’s Healthcare Improvement (CCHI).

Craig Mitton, MSc, PhD Craig is a Senior Scientist at the Centre for Clinical Epidemiology and Evaluation and an Associate Professor in the School of Population and Public Health in the Faculty of Medicine at UBC. Craig’s research focuses on the application of health economics to impact real-world decision making in health organizations. He is a member of the International Society of Priorities in Health Care and serves as an Associate Editor for the journal BMC Health Services Research. In addition to an active applied health services research program he regularly consults with governments and health care organizations both in Canada and abroad.

Christina Krause, MSc Christina has a strong background in improvement work leading collaboratives on topics such as medication reconciliation, prevention of surgical site infection and MRSA , and improved care for acute myocardial infarction. A certified Faculty member with the Institute for Healthcare Communication, she also served as a key member of the National Working Group which developed Pan-Canadian Disclosure Guidelines. In her current role, Christina has a particular interest in social movement theory and the mobilization of energy to facilitate large-scale change. Christina is currently an EXTRA Fellow with the Canadian Health Services Research Foundation.

Tanis Rollefstad, RN, BN, MA candidate Tanis has a long standing clinical background in cardiac care and has devoted the last 10 years on Quality Improvement and Patient Safety. She has taught extensively at National and Provincial programs such as the Quality Improvement School in Saskatchewan with a specialty in reliability science and spreading change through a health system. She is pursuing a Masters in Communications and Technology Program at the University of Alberta and is the Safety and Improvement Advisor of the Western Node with Safer Healthcare Now!

Allison Lamsdale, MASc Allison is a Human Factors Specialist with strong experience in applying engineering and psychology principles on a variety of large and small projects relating to healthcare. She currently works part-time at Vancouver Coastal Health as a Human Factors Specialist in Quality and Patient Safety, and independently consults on Human Factors principles to a variety of industries in Vancouver, British Columbia, and across Canada. Committed to safety and quality of care, Allison’s goal and motivation is to introduce and incorporate human factors engineering principles into healthcare in British Columbia.

Dr. Martin Wale, BMed Sci, BM BS, Dp Bact, FR CPath, MBA, RD Martin joined VIHA after leaving the NHS (UK) in May 2009 to take his current position with responsibilities of an Island-wide portfolio including quality and patient safety, infection prevention and control, accreditation, privacy and ethics. He was awarded a Fellowship scheme at the NHS Institute for Innovation and Improvement while working on patient safety and Clostridium difficile associated disease (CDAD). Martin achieved Distinction in his Executive MBA in 2008 and became responsible for all aspects of medical performance in a large acute Trust servicing three health regions in the NHS as Executive Medical Director.

Shari McKeown Quality Leader BC Patient Safety & Quality Council Shari is a Registered Respiratory Therapist with 15 years of critical care experience, and has received a fellowship designation from the Canadian Society of Respiratory Therapists for her significant contribution to the profession. She joined the Council in January 2011 as Quality Leader, supporting the provincial implementation of Clinical Care Management in the prevention of venous thromboembolism as well as best practice for the care of critically ill patients. She has completed training in Quality Improvement methodology from the Institute of Healthcare Improvement, and received her Master's of Arts in Learning & Technology from Royal Roads University in 2011. She has led numerous patient safety and quality improvement initiatives and shared that knowledge with an extensive multidisciplinary network of care providers across North America.

Heidi Johns Quality Leader BC Patient Safety & Quality Council Heidi is a Quality Leader with BCPSQC and is supporting the congestive heart failure strategy for Clinical Care Management (CCM). She is also involved with the integration, quality coaching, and mentoring at Northern Health with the Quality and Innovation Department. Heidi has more than 12 years’ experience in health care, much of it working in vocational rehab for Mental Addiction Services at Northern Health. More recently, Heidi has been working coaching primary care integration and quality improvement as well as participating in the development of techniques that involves the spread of quality improvement concepts and tools to point of care providers. In addition Heidi is passionate about discovering ways to meaningfully engage communities and patients in the transformative changes in healthcare. She holds a Bachelor of Arts degree in Political Science, from the University of Northern BC, with a focus on health policy. |

Melanie Rathgeber, MA Melanie has a breadth of experience facilitating innovative and system-wide collaborations to improve quality. Specializing in measurement, Melanie has worked with national, provincial and local initiatives to define indicators and measurement systems. Melanie's main focus is appropriate data display and ensuring indicator results are meaningful to all levels of staff and leadership. In previous roles with Saskatchewan's Health Quality Council and the the BC Patient Safety and Quality Council, Melanie led projects in a variety of care areas including surgery, ICU, wait times for cancer care, and chronic disease care in the community. She has also been instrumental in designing education programs for staff and physicians. Melanie now divides her time between consulting in the health care and social services sectors and works as Quality Leader for BC Mental Health and Addictions Services.

Erin Langin Quality Improvement Consultant Erin has experience supporting quality teams to apply a range of improvement tools, methodologies and theories to improve care and service delivery. These projects include primary care, acute care and outpatient settings. She has extensive knowledge of Quality Improvement Science, systems thinking, Advanced Access and the Model for Improvement. Erin is a certified ASQ Lean Six Sigma Green Belt. Erin graduated from the MBA program at the U of S with a specialty in Health Services Management. Upon graduation, Erin began work the Health Quality Council in Saskatchewan. Erin was involved with several provincial initiatives, including the Chronic Disease Management Collaborative, Clinical Practice Redesign and the Releasing Time to Care Program. Currently, she provides Coaching support to Quality Academy Program participants and Saskatchewan’s Quality Improvement Consultant Program participants. Erin resides in Regina, Saskatchewan. |

Wrae Hill, MSc, RRT,
FCSRT Director Patient Safety and System Safety Interior Health Wrae has a breadth of clinical knowledge and skill in the area of patient safety and quality improvement, and has a particular interest in the area of resilience and high reliability in health care. Wrae is a fellow of the Canadian Society of Respiratory Therapy with a MSc in human factors and systems safety from the Center for Complexity and Systems Thinking at Lund University in Sweden. Wrae has certification in training of root cause analysis and disclosure, and co-developed a course for patient safety investigators. Wrae's current role includes regional responsibility patient and systems safety for Interior Health.

Bruce Harries, MBA Bruce co-founded Improvement Associates in 2000 and has worked primarily on the improvement of healthcare. He is Collaborative Director for the Canadian Collaborative to Improve Patient Care and Safety in the ICU, and advises on a number of healthcare improvement initiatives. He is a graduate of Trent University, the Banff School of Advanced Management and holds an MBA from IMD in Lausanne, Switzerland. |
Dan Horvat, MD, MCFP, BSc Dan has a long standing interest in sustainably improving healthcare. His experience includes roles such as family physician, medical administrator, academic, regional health board member and consultant. His interest in health system improvement has led him to explore the local and international community in search of best practices. A focus of his research involves distilling the key, context independent activities that support highly effective healthcare systems. Dan has also worked extensively in areas including primary care and the use of measurement and information technology in improvement.

Cathy Weir, MAEd Cathy is employed as the Director, Quality Improvement and Patient Safety with Fraser Health. She is responsible for leading the implementation of organization-wide quality and patient safety strategies such as the Patient Safety and Learning System (PSLS ) and a performance accountability system for quality and patient safety based on the BC Patient Safety & Quality Council’s Health Quality Matrix. Cathy has a critical care nursing background, a Masters in Adult Education, and has led quality improvement initiatives at the organizational level for over 15 years.

Andrew Wray, BSc Andrew is responsible for the development of learning opportunities in quality and patient safety for BC healthcare providers, patients and the public, as well as supporting work towards improving relationships and communication between patients, families and healthcare workers. In partnership with Vancouver Coastal Health and Providence Healthcare, Andrew helped introduce the Global Trigger Tool to health authorities in BC and maintains a Community of Practice for continued collaboration. Prior to joining the Council, Andrew spent several years working in the biotechnology industry, the BC Cancer Research Centre and most recently, the BC Ministry of Health Services where he focused on infection prevention and control.
