Quality Academy
Enrolment Information
To register for the Quality Academy, simply follow the directions below.Quality Academy Cohort 4 is now full!
Application Process
| 1. | Work with your Direct Supervisor to identify an Executive Sponsor and begin initial exploration of your quality project ideas with them. |

| 2. | Download the Application Form and Executive Sponsor form from the BCPSQC website. |

| 3. | Complete the Application Form and click “Submit Form” to apply. You can also save a copy of the form. |

| 4. | Have your Executive Sponsor and Direct Supervisor sign the Executive Sponsor Form and send to the BCPSQC via fax at 604.668.8220 or as a scanned pdf document via email to msahota@bcpsqc.ca. |

| 5. | Once your Application Form and Executive Sponsor Form has been submitted, you will receive an email confirming receipt. Applications will not be considered until all materials are completed. |
Notification of Acceptance
Starting December 5, 2011, participants will be notified of their acceptance to the Quality Academy. At this time, additional details regarding participation will be sent including the process for submitting program payment. Completion of the registration process will confirm acceptance of a seat in the Quality Academy starting in April 2012.
