Six Thinking Hats

The difference between poor and effective teams lies not so much in their collective mental equipment but in how well they use their abilities to think together. The Six Thinking Hats technique helps actualize the thinking potential of teams.

Disclosure of Unanticipated Medical Outcomes (DUMO)

This session is aimed at providing an understanding of the process of disclosure - an open and honest conversation that occurs when an event occurs in a patient’s health care, whether it results from the national progression of an underlying medical condition, inherent risks of an investigation or treatment or from a preventable adverse event. This session will provide an overview of the Canadian Disclosure Guidelines, the needs of patients and families during the disclosure process as well as exploring a framework for disclosure conversations.

Patients for Patient Safety Canada (PFPSC) is a patient-led national organization committed to bring the perspective of patients and families to healthcare organizations and systems engaged in safety improvement. PFPSC is comprised of volunteers across Canada who, as patients with safety-related experiences, are bringing their perspectives to a forum in order to
- provide a collective credible voice reflective of diverse perspectives for patients across Canada
- promote best/leading practices in patient centered care and patient safety
- advocate for the patients perspectives in all healthcare agendas
- work collaboratively with patients, healthcare providers, health organizations and other agencies to accelerate efforts aimed at improving patient safety.

Hear tips and lessons from teams across the country who have implemented the Surgical Checklist in their hospitals. Presenters, Marlies van Dijk and Tanis Rollefstad from Safer Healthcare Now!, who facilitate the Safe Surgery Saves Lives Checklist Action Series. During this session they will summarize what providers, staff and improvement teams have identified as challenges and strengths when it comes to implementing the Surgical Checklist. They also comment on what has been learned recently about measurement and implementation of the checklist, and what value the checklist can bring to the teamwork and culture of the OR.

The Bedside Observer Project

The presentation describes an innovative approach to learning from safety events at BC Children's Hospital. The Bedside Observer Project involved family members in reporting safety incidents and near misses using a system integrated with the Patient Safety Learning System. This unique project provided valuable information for learning from safety events while engaging families as a valued part of the care team.
