Excellence in Quality
Awarded to a team or initiative focused on improving care in each of the following areas. Click the category titles below to download the online submission forms.
Excellence in Quality: Staying Healthy– Preventing injuries, illness and disabilities
Excellence in Quality: Getting Better– Care for acute illness or injury
Excellence in Quality: Living with Illness – Care and support for chronic illness and/or injury
Excellence in Quality: Coping with End of Life – Planning, care and support for life-limiting illness and bereavement
Excellence in Quality: Theme Award - Across the Province- Spreading change for improvement on a larger scale, across the province

Awarded to an individual who has demonstrated significant leadership in the field of quality or patient safety in British Columbia, with a particular emphasis on involvement in the spread of success across the Province.
Please review the online Adobe PDF submission form and prepare your responses as clearly and concisely as possible.
- Open the Online Submission Form
Click on one of the links above to open the submission form that best suits the team, initative or leader you would like to nominate. You can save the Adobe PDF submission form to your computer if you would like to continue working on it later.
- Enter Your Responses
Enter your responses by typing or copying and pasting into the fields on the form. Note that there is a limitation on the number of characters that can be entered in each field to help ensure consistency among the submissions and scoring. Some formatting, including bullet points, can be copied and pasted into the Adobe PDF submission form.
- Submit Your Nomination
Make sure to save a copy of your submission to your computer for your records. You can submit the Adobe PDF submission by clicking the “Submit” button at the top of the form, or you may email it to awards@bcpsqc.ca as an attachment.
- Appendices
Appendices are limited to a maximum of three pages.
To ensure consistent scoring, the submission form includes limitations on number of words that can be included for each section. Similarly, only three pages of attachments or appendices will be accepted. Please ensure your responses are as clear and concise as possible.
Appendices are to be submitted by email to awards@bcpsqc.ca. Please include “Awards submission” in the subject line, along with your name and the award category.
To help protect the environment, please do not send hard copies of your submission form or the attachments.
An email confirmation will be sent after your submission is received. If you do not receive an email confirmation within 48 hours please contact Kerianne Poulsen at 604-668-8225.
